Project Name: Lipid Optimisation Project

Project Summary:

The purpose of this Collaborative Working Project (“CWP”) is to improve the quality and equity of care for eligible patients with established atherosclerotic cardiovascular disease (“ASCVD”) who are under the care of the CW Partner and whose lipid management may require review in accordance with national and/or local guidance.

The CWP will support the CW Partner to identify eligible patients, offer them an appropriate clinical review and, where clinically indicated, optimise their care according to individual patient need and applicable national and local guidance.

The CWP will be led by the CW Partner’s Clinical Transformation Lead. Novartis will provide funding for the following additional CWP Roles:

  • Independent prescriber, band 8a, 0.18 FTE, 352 hours.

  • Healthcare assistant, band 3, 0.07 FTE, 130 hours.

  • Administrator, band 2, 0.10 FTE, 208 hours.

The CWP Roles will support the CW Partner to:

  • Identify eligible patients with established ASCVD whose lipid management may require clinical review.

  • Contact eligible patients and offer them an appropriate appointment.

  • Undertake clinical reviews and discuss appropriate next steps with patients through shared decision-making.

  • Optimise care where clinically appropriate and in accordance with applicable national and local guidance.

  • Support relevant primary care healthcare professionals’ awareness of the agreed ASCVD lipid-management pathway, referral arrangements and follow-up responsibilities.

The CW Partner will employ or engage, manage and clinically supervise the CWP Roles and will remain responsible for all patient-facing activity, clinical governance, prescribing decisions and data management. Novartis will provide the agreed financial contribution and non-financial project-management support. Each party will make a significant contribution to the delivery of the CWP.

The CWP includes 26 weeks (6.5 months) of clinical activity. All patient identification, clinical assessment and prescribing decisions will remain solely the responsibility of the CW Partner and will be made independently, according to individual patient need and applicable national and local guidance. The CWP does not require or encourage the use of any particular medicine.

Expected Benefits:

PATIENTS:

  • Potential for improved patient experience through a more coordinated primary care lipid-management pathway.
  • Potential reduction in waiting times for eligible patients requiring review.
  • Earlier identification and review of eligible patients with established ASCVD whose lipid management may require optimisation.

  • Improved access to appropriate treatment options, based on individual clinical need and applicable national and local guidance.

CW PARTNER:

  • Additional capacity to identify and clinically review eligible patients with established ASCVD.
  • Improved use of agreed referral, review and follow-up processes within the local lipid-management pathway using the protocol developed during this CWP.

  • Identification of eligible patients whose lipid management may otherwise not have been reviewed.

  • Service activity and outcomes data to support evaluation and future service planning.

NOVARTIS:

  • Improved understanding of the operational challenges affecting the identification, review and management of patients with established ASCVD in primary care.
  • Learning from the CWP’s implementation and measured outcomes, which may inform the development of future collaborative working projects addressing similar healthcare-system needs.

  • Strengthened experience of working transparently and ethically with healthcare organisations to address documented patient and NHS service needs.

  • Novartis is likely to see positive outcomes as a result of the CWP. As more patients are reviewed there is a potential for greater utilisation of Novartis’s licensed medicines, in line with national and local guidelines, where appropriate.
